Iron deficiency is one of the most common nutrient deficiencies in young children, both in Australia and worldwide. And yet it's one of the most frequently missed, because the early signs don't always look like what you'd expect.
Most parents associate low iron with pale skin or tiredness. And yes, those can be signs. But in toddlers and young children, low iron often shows up differently: as irritability, poor appetite, frequent illness, disrupted sleep, or slow weight gain. These are things parents (and sometimes doctors) put down to "just being a toddler" or a phase.
As a nutritionist and mum, I want to walk you through what to look for, why iron matters so much in these early years, and what you can actually do about it, especially if your child won't eat red meat.
Why iron matters so much for growing kids
Iron isn't just about preventing anaemia. It plays a role in almost every system your child's body is building right now:
What iron does for your child
A 2024 review published in Nutrients highlighted that low iron status in children isn't just about fatigue. It can also be associated with changes in behaviour, attention, and sleep quality. Iron also plays a key role in brain chemistry and nervous system regulation, including the production of neurotransmitters like dopamine.
As soon as I know something may affect my child's mood and sleep, I'm onto it. And iron is one of those nutrients where the effects of deficiency can start showing up well before a blood test flags full-blown anaemia.
Signs to watch for
Iron deficiency develops gradually. The early stages (low ferritin stores) don't always show up on a standard blood test if your GP only checks haemoglobin. Here are the signs I tell parents to look for:
Common signs of low iron in children
If any of these sound familiar, it's worth asking your GP for a full iron panel, not just a haemoglobin check.
Ask for the full iron panel
This is something I feel strongly about. A lot of parents are told their child's "blood test came back fine" when the GP has only checked haemoglobin. By the time haemoglobin drops, iron deficiency is already well established. Catching it earlier means checking ferritin (stored iron), which many standard blood tests don't include unless you specifically request it.

If iron stores continue to drop, further investigations should be made by a health professional to rule out potential causes. But for many children, especially fussy eaters and those avoiding red meat, the cause is simply not getting enough iron from food.
A note on blood tests and little ones
Blood tests can be genuinely traumatic for young children, and I don't say that lightly. If your child is showing some of the signs above but is otherwise growing and developing well, it's worth having a conversation with your GP about whether a blood test is the right next step or whether it makes more sense to observe clinical signs and symptoms, focus on increasing iron-rich foods and gentle supplementation, and reassess from there. There's no one-size-fits-all answer here. Trust your GP, but also trust your gut as a parent.
The problem with most iron supplements
If you or your child has ever been prescribed iron tablets, you probably know the story: constipation, nausea, black stools, tummy pain. These side effects are so common that many parents (and kids) give up on supplementation altogether.
Here's why this happens. Most iron supplements contain iron salts like ferrous sulfate, ferrous fumarate, or ferrous gluconate. These synthetic forms behave differently in the gut. They're poorly absorbed, which is why they're often prescribed in high doses (60 to 100mg+), and the unabsorbed iron sitting in the digestive tract is what causes the discomfort.
The hepcidin problem
When iron is taken in high doses, the liver releases a hormone called hepcidin, which blocks iron absorption in the gut and prevents excess iron entering circulation. Research shows doses of 60mg or more of elemental iron increase hepcidin for up to 24 hours, reducing the absorption of later doses. This is why taking high-dose iron every second day may actually improve absorption compared with daily dosing.
There's also the cofactor issue. Iron metabolism depends on several nutrients working together. Deficiencies in vitamin C, vitamin B12, folate, vitamin A, copper, or magnesium can impair iron utilisation even if intake is adequate. A standalone iron tablet doesn't address any of these.
Why whole-food iron is different
Forms of iron that are often better tolerated include iron bisglycinate and food-based iron (haem iron). Haem iron, which comes from animal sources, is significantly more bioavailable than the non-haem iron found in plant foods and synthetic supplements. Your body recognises it, absorbs it more efficiently, and it doesn't cause the same gut issues.

The challenge, of course, is that many toddlers and young children won't eat enough of these foods consistently. If your child has decided that red meat is off the menu (along with anything green, anything new, and anything that touched something else on the plate), food alone may not be enough to maintain healthy iron stores.
How to give it to kids who won't swallow capsules

Easy ways to sneak it in
Stir into smoothies, porridge, or yoghurt. Mix into pancake or waffle batter. Add to pasta sauce, bolognese, or soup. Blend into banana bread or muffin batter. For adults, simply swallow the capsules with food.

A note about zinc and iron timing
